Plan and Ask, enforced rather than requested
Plans from Claude, Codex and ACP get a card in the transcript instead of being discarded, and Ask is a read-only mode each backend actually enforces.
Every backend can produce a plan and each expresses it differently. Realm was dropping the structure and keeping the prose. Plans now carry into the transcript as a card, from all three families.
Ask is read-only. It is enforced by the backend rather than asked for in a system prompt, and where a backend cannot enforce it, Realm does not offer it.
Offer a capability only where its owner has said it exists. A control offered on a guess has one outcome: a refusal.
