Skills, discovered and @-mentioned

Realm finds the skills already installed on the machine and lets a session reach one by name from the composer.

Skills were already on disk, spread across the directories each CLI uses. The discovery service reads all of them and reports scope (user, project, or system), so it is clear which copy of a name is in play.

In the composer, @ reaches a skill by name. Realm injects per invocation rather than writing to the CLIs' own configuration; ~/.claude, ~/.codex and ~/.cursor are read-only as far as Realm is concerned.

A skill switched off in Settings is not a chip in the bubble, so a transcript never claims a skill was in play when it was not.
